Chapter 2544 Time Control is Important

The First Affiliated Hospital of Zhongshan Medical University must have the technical strength to do this, so there is no need for them to fly over. The doctor who will be responsible for removing the donor's heart during the operation is Dr. Ye Chuanguang.

"Do the doctors there have any requirements for us?" Ye Chuangguang asked.

While Xie Wanying was talking on the phone, she took out the ballpoint pen from her hand.

Seeing this, Wei helped her take out her notebook so that she could write on it.

Xie Wanying wrote down the matters she needed to coordinate with the other party and asked Teacher Cheng to review them.

Doctor Cheng Yuchen looked at it and nodded.

After getting the teacher's consent, Xie Wanying said, "First of all, the distance and time of transportation of the donor heart are quite long, so some preparation work requires your cooperation, Teacher Ye."

"You're welcome." Ye Chuanguang said frankly, "If you hadn't called us, we would have wanted to ask you what your plan was. Do you need us to refill the donor heart with cardioplegia solution from the aortic root to preserve it?"

"Yes." What a great teacher! He understood it almost instantly without any further explanation.

...

...

"But you have to understand that the cold ischemia time of the heart should not exceed five or six hours." Ye Chuangguang kept reminding people not to think that everything will be fine if they take more preparatory steps, as it will only delay the time a little.

Cold ischemia can be said to be a technical term in organ transplantation technology. Before talking about cold ischemia, let's first talk about another similar technical term: warm ischemia.

The period of time between the organ being removed from the donor and being stored cold after the heart stops beating is called warm ischemia. Cold ischemia corresponds to the period of time between the organ leaving the donor and being transplanted into the recipient, which mainly refers to the transportation time.

Each organ has an upper limit for warm ischemia and cold ischemia. For example, the upper limits for warm ischemia and cold ischemia of the heart are the shortest, which are ten minutes and eight hours respectively. The ten minutes and eight hours here are super upper limits, which are equivalent to a big adventure. No one can easily take this risk. Therefore, Dr. Ye said that the cold ischemia time should not exceed five or six hours. The warm ischemia time of the donor heart removed in their hospital will be controlled to within five minutes.

What is the matter with the injection of cardioplegia? Here again, when the organ is removed from the donor, the cells are not dead and continue to metabolize. Since the blood flow is interrupted and there is no blood nourishment, the removed organ will inevitably suffer metabolic damage due to self-consumption. It can be imagined that the warm ischemia period is the most damaging to the donor organ, and skilled professional doctors are needed to do this to shorten the operation time as much as possible.

How to protect the donor heart during the cold ischemia stage? From a medical technical point of view, either the ideal state is to use a machine to simulate the normal human body to continuously perfuse the donor heart, so as to minimize the damage. The problem is that this is simply not possible during long-distance transportation, so the only option is to use other methods to reduce the metabolic rate of the donor heart and reduce damage. Therefore, the general organ preservation solution must contain low temperature and other ingredients to inhibit organ cell metabolism.

The energy consumption of the heart is mainly composed of the movement of cardiac contraction and relaxation and the metabolism of myocardial cells. The former accounts for 90% of the total consumption. Using cardioplegia solution can stop the mechanical activity of the heart, and low temperature can reduce the metabolism of myocardial cells. The combination of the two is equal to one plus one, and the effect of reducing consumption is doubled.

Protecting the myocardium is one aspect.

On the other hand, because the heart is not like the liver and has a special structure, the donor heart and the recipient heart require multiple port anastomosis.
